Additional Documentation Required for Minors:

-Current State issued photo ID for client (school ID, drivers license/permit, passport, non driver ID card, military ID)

-State issued photo ID for parent or legal guardian

-State issued or other bonafide form proving legal guardianship (birth certificate, court or state issued guardianship papers)

*all documents required for minors, no exceptions made.
*Some procedures not available to minors.

Procedures available to Minors: (with above listed documentation)

-age 13: earlobes, basic ear cartilage only

-age 14-15: basic piercings including: navel, eyebrow, nostril, ear

-age 16-17: basic oral piercings including tongue, lip, labret. Basic piercings including navel, eyebrow, nostril, ear.

Procedures not available to minors:
-All piercing not listed above including surface to surface, complex ear cartilage piercings, and piercings of an adult nature.
-Procedures such as: tattooing, scarification, dermal punching, large gauge piercings, large gauge stretching
